Title IX and Sex Discrimination Complaint Form

As required by Title IX and its implementing regulations, 34 C.F.R. Part 106, NUHS does not discriminate on the basis of sex in its educational programs and activities. Additionally, the university is committed to providing a work and student environment that is free of sexual harassment, and will not tolerate sexual harassment activity by any university employee, student, or third party. NUHS will act promptly and impartially to address claims of sexual misconduct and discrimination and remedy the effects of discrimination. Should you feel that you have been sexually harassed or have witnessed a person being sexually harassed, please submit your complaint utilizing the NUHS Title IX – Discrimination Complaint Form.
Once this form has been received and signed by the NUHS Title IX Coordinator, the complaint is considered properly received and noted by the university. During the investigation process, a copy of the completed form will be provided to the originator.
Submission of this form authorizes the NUHS Title IX Coordinator, or their designee, to conduct inquiries or investigation procedures as needed with respect to the investigation/resolution of this complaint. It is understood that information regarding the complaint may be shared, as needed, with applicable university officials in order to acquire sufficient information with respect to the investigation as well as any follow-up activities that may be required in relation to the university’s response to the complaint. The originator also authorizes the university to use whatever information may be obtained with respect to this complaint in any legal or formal grievance proceedings that may involve the issues contained herein.
